Case Management

India Home provides personalized case assistance, information, and referrals to help seniors access the services, benefits, and entitlements they may otherwise struggle to obtain on their own. Many of our members are either unaware of available public benefits or face barriers in navigating application systems due to limited English proficiency, as well as challenges like lack of access to transportation or digital tools such as computers.

Our case managers work directly with seniors at our centers to understand their needs, identify eligible benefits, and connect them with appropriate services through targeted referrals and advocacy support. A personalized service plan is developed for each client, and when needed, our team advocates on their behalf to help ensure fair access to rights and entitlements. Support is offered in Bengali, Hindi, and English to ensure services are accessible and culturally responsive.

Some of the situations we provide case management for are:

Benefits assistance – Help with applying for SNAP (Supplemental Nutrition Assistance Program), SSI (Supplemental Security Income), housing assistance, disability benefits, and other eligible programs.

Bill support & disputes Help understanding and resolving issues related to internet, cable, and medical bills.

Citizenship and immigration support – Guidance on naturalization, green cards, re-entry permits, visas, and related immigration concerns.

Document review & interpretation Assistance reading and explaining official letters, notices, and important documents

Employment guidance – Support with questions about senior employment programs and income support opportunities.

Health Insurance Navigation Support with Medicaid applications, renewals, address updates, and understanding eligibility for Medicare, Medicaid, and ACA coverage.

Mental health support Our case managers are certified in Mental Health First Aid and provide supportive assistance for seniors experiencing anxiety, depression, loneliness, and social isolation.

Transportation assistance – Assistance with applications for Reduced Fare MetroCards and Access-A-Ride services.
